Description:
Cardiac rehabilitation is a supportive program designed to empower patients to recover and rebuild their health after a cardiac event or support those living with a heart condition.
Our program is delivered in collaboration with the Huron Perth Ontario Health Team and supports patients both within and outside of our Family Health Team.
It is designed for men and women with known heart disease, including but not limited to those who have had a heart attack, angina, angioplasty, or heart surgery. If you have recently experienced a cardiac event or been diagnosed with heart disease, you may be a good candidate for the program.
You will work with our multidisciplinary team to complete an individualized program that includes education, supervised group exercise, and personalized goal setting. The program provides a safe and supportive environment to help you regain confidence with physical activity and in your daily life.
As part of the program, participants receive a 12-week membership to the St Marys YMCA to support participation in the supervised exercise component.
Education focuses on heart-healthy lifestyle changes, including physical activity, nutrition, medication adherence, and managing cardiovascular risk factors. Participants are supported to make sustainable changes that improve long-term heart health and quality of life.
Cardiac rehabilitation is a safe and effective way to improve heart health, reduce symptoms, and lower the risk of future cardiac events. It also supports your return to an active social and/or work life and promotes overall well-being.

- Patient of Happy Valley Family Health Team: No
- Appointment Required: Yes
- Also: Clinical diagnosis of cardiac event or heart condition is required.

If you have been diagnosed with a heart condition, or have had a heart event, please speak with your family doctor to discuss whether or not you would benefit from a referral to our Cardiac Rehabilitation Program.
