Diabetes Program

Description:

Diabetes is a disease that affects your body’s ability to make insulin or properly use insulin. Insulin is a hormone produced by your pancreas.

Insulin’s role is to regulate the amount of glucose (sugar) in the blood. Blood sugar must be carefully regulated to ensure that the body functions properly.

Our program provides routine monitoring for our diabetic patients. Our goal is to improve our patient’s health and reduce the potential for complications associated with diabetes. Education and support is available from our multidisciplinary team to assist in managing diet, exercise and medication.

If you are newly diagnosed with diabetes or managing an active diagnosis of diabetes, speak with your Family Doctor about how frequently you should book a diabetic visit.

During your diabetic appointments, you may continue to see your Family Doctor or one of our Nurse Practitioners for an assessment and review of your individualized plan of care. It is possible that your diabetes care provider may also suggest follow up with our Pharmacist or Registered Dietician.
